Live Well Boone County is a community health assessment project convened by Columbia/Boone County Public Health & Human Services. A community health assessment, or CHA, collects and analyzes data to understand the health of a given community. The assessment harnesses the unique insights, expertise, and perspectives of individuals and communities directly through community feedback and engagement to tell the community’s story.

Community input is paired with data on the status of a community, such as demographics, health status, quality of life, social and economic factors, and health inequities. Communities can then use data compiled in the CHA to identify "upstream" factors that may not have been explored before to gain a better understanding of what impacts health in their community. Sharing information from the CHA can bring community members together to build a healthier community and prevent disease.

Since 2018, results from the previous community health assessment have helped inform nearly $42 million in health and social service programs, which served over 415,000 total people in Boone County.
