What is Live Well Boone County?
Live Well Boone County is a group of organizations across Boone County that work together to conduct a community health assessment and improvement plan. There is a wide range of organizations that are part of Live Well Boone County including health care providers, non-profits, school districts, faith-based organizations and more.
Who can take the survey?
The survey is open to Boone County residents ages 12 and up. Minors must have parent/guardian approval to take the survey.
Where can I take the survey?
The survey is available online and at several locations in the community. The survey is open until April 28, 2023.

Yes, information collected in this survey is anonymous and used only in combination with others participating in the survey. No personal identifying information, such as your name, date of birth, or phone number, will be collected. Your responses cannot be traced back to you.
Why do I need to include my demographic information and zip code?
Information like demographics and zip codes help to describe populations and their characteristics, and help us to look deeper into issues identified in the survey.
How will the results of the survey be used?
A community health improvement plan will be developed by combining the survey results with other data gathered in the assessment. In the past, survey results have informed local organizations to allocate funds directly to community health projects.
What is Live Well Boone County?
Live Well Boone County is a group of organizations across Boone County that work together to conduct a community health assessment and improvement plan. There is a wide range of organizations that are part of Live Well Boone County including health care providers, non-profits, school districts, faith-based organizations and more.
What is a community health assessment?
Community health assessments, or CHAs, collect and analyze data about the health of a community. Data can be feedback from community members through methods such as surveys and focus groups. In addition, data on health behaviors, rates of chronic disease, available services, and quality of life are also collected. The result is comprehensive information about community strengths that can be used to address identified health issues.
What is a community health improvement plan?
Community health improvement plans, or CHIPs, are long-term plans developed based on the results of community health assessments. The plan helps health and community organizations set priorities and coordinate resources. Additionally, a CHIP can be used to develop communitywide health policies.
How will the results of the survey be used?
The results of the survey will be integrated with other ongoing data collection tools to identify themes and determine key issues in the community. These data will help the Live Well Boone County steering committee narrow down strategic issues, why the issues are important to the community, key indicators related to the issues, and potential solutions.
